Student Information Form This is a fillable form, which means you can type in your information and then print it out.
Proof of Residency Procedures Provide one document from Column A and one document from Column B and fill out the right-hand side of the paper.
Residency Affirmation This form must be completed, signed by both parties, notarized, and returned to the school. Please note that you MUST sign in the presence of a notary.
Guardianship Status Form Provided English and Spanish options. Fill out and sign.
Utah School Immunization Record Complete the form.
Homeless Assistant Act Questionnaire Answer the two questions on the form.
